All change requests must be accompanied by payment of the $25 change fee (check or money order ONLY) + the price difference, if any, for event category.
For example, if you are a USAT member and are switching from the Sprint Open Age Group ($155) to the Relay category ($285) then you will need to pay the $25 change fee, plus the $130 difference in categories, for a total of $155.

• Incomplete requests will not be considered.
• If you are switching to a relay team, you will need to include the completed and signed entry forms for the new team members.
• All change requests must be submitted and postmarked by July 1, 2010. NO EXCEPTIONS.
