Required Forms For Pro Triathletes

We work very hard to highlight the Professional Triathletes participating in the Life Time Tri Chicago.  To help facilitate these efforts, please complete the attached form and email chicagotripros@yahoo.com or fax to 773-404-6591.

Prize Money For Pro Triathletes

Life Time will be awarding a total of $_____________ in prize money. The prize money will be divided equally between men and women as follows:
     1st Place    $10,000          6th Place     $ 2,000
    2nd Place    $  6,000          7th Place     $ 1,800
     3rd Place    $  4,500          8th Place     $ 1,200
     4th Place    $  3,000          9th Place     $    900
     5th Place    $  2,500        10th Place     $    600 

GATORADE PRIMES 

We will be awarding $2,000 in Gatorade Primes as follows:

Swim Prime: $500 to the 1st male and 1st female Pro to pass under the Swim Finish banner.

Bike Prime: $500 to the 1st male and 1st female Pro to pass by the Bike-In banner at transition.

U.S. TAX REGULATIONS
Cash awards are subject to US taxes. You must provide proof of a valid US Social Security number and sign an affidavit when you pick up your race packet to avoid the 30% US IRS withholding. You will be required to fill out an appropriate tax form (W-9, W-8, etc.) before collecting your race packet.

PAYMENT OF PRIZE MONEY
Checks for prize money awards will be mailed to winners within three weeks of the race. Please confirm we have both your correct mailing and e-mail addresses.

ADDITIONAL AWARDS 

Travel: We will be awarding a certificate for a round-trip ticket on American Airlines (continental US only) to the 1st Place Male and 1st place Female Pro finishers. Certificates may only be used on American Airlines flights, and the tickets expire on the date listed on the certificate. Certain restrictions apply. Certificates will be mailed to winners with the check for their prize money.

Accomodations: Accommodations: a) Male and Female winners of last year’s “Life Time Fitness Triathlon Series Race to the Toyota Cup” (registered for the 2012 event) receive a free two-night stay race weekend at the Hilton Chicago, b) top three male and female finishers of the 2011 Life Time Tri Chicago (registered for the 2012 event) will receive a two-night complimentary hotel stay for the 2012 race weekend at Hilton Chicago.  Certain restrictions apply. You will be contacted by event staff for reservation information.
